To request an angel, fill out the form or write to the mailing address below. Please include name, address, phone and ethnicity, along with a little information about the recipient. Incomplete forms will not be honored. Due to low funding at this time, we must ask for a donation to cover the shipping costs only. Not all angels will be handpainted ceramics, as we have outgrown our ability to keep up with the demand. Angels of Destiny Ministries makes no guarantee of availability, delivery or product warranty.
